I received this chain email from a friend and instead of forwarding it along and polluting the email-o-sphere with my answers, I figured I would just post it on my blog instead.  I took a quick look around the blogosphere and learned that it is already an age old meme as found in at least one, two, three, four other blogs.  So here goes my answers (combining all the questions I've collected):
